Application Package Steps
All 3 steps must be completed before Lag B’Omer, 18 Iyar – May 3rd, in order for your application to be
considered in a timely fashion.
Step 1:
Complete the preliminary on-line form. Click the following link to access the On-line Application form
Step 2:
Download and print the main application package, complete all portions, scan and email to chayamushkaseminary@gmail.com or mail to 5115 Vezina, Montreal, Qc H3W 1C2 Canada
Step 3:
Submit the application Fee. The PDF credit card form is available for download within the main
application package, in step 2 above, or can be accessed with the following link: Application Fee Payment
